Summary
Barbiturates produce marked CNS depression that adds to the sedation of brexpiprazole. Butabarbital is also an enzyme inducer and may lower brexpiprazole exposure over time, reducing efficacy.
What this interaction means in plain words
Brexpiprazole and Butabarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive CNS depression, plus induction of CYP3A4 by barbiturates increasing brexpiprazole clearance.
What to do
Monitor sedation and respiratory status and watch for loss of antipsychotic effect. Increase brexpiprazole dose as needed per labeling for CYP3A4 inducers, or choose an alternative hypnotic.
What to ask your pharmacist
