Summary
Butalbital is a barbiturate and can add to the sedating effects of brexpiprazole, causing drowsiness, dizziness and impaired coordination. Butalbital also induces CYP3A4, which may modestly lower brexpiprazole exposure.
What this interaction means in plain words
Brexpiprazole and Butalbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive CNS depression, plus CYP3A4 induction by barbiturate that may reduce brexpiprazole concentrations.
What to do
Avoid regular combined use where possible; if needed, monitor for sedation and loss of antipsychotic efficacy and consider increasing the brexpiprazole dose when butalbital is used chronically.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
