Summary
Pentobarbital induces CYP3A4 and can substantially lower brexpiprazole concentrations, risking loss of psychiatric control. Both drugs are also sedating, so additive drowsiness and respiratory depression may occur.
What this interaction means in plain words
Brexpiprazole and Pentobarbital can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
CYP3A4 induction by the barbiturate increases brexpiprazole clearance, plus additive CNS depression.
What to do
With prolonged barbiturate use, increase the brexpiprazole dose gradually toward double the usual dose over 1 to 2 weeks and monitor response. Reduce brexpiprazole back to the usual dose after the inducer is stopped, and monitor sedation.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
