Summary

Chronic inflammation raises cytokines that suppress CYP450 enzyme formation. IL-17 receptor blockade with brodalumab may reverse this suppression, so theophylline clearance could rise and serum levels fall. Because theophylline has a narrow therapeutic index, the effect could be clinically relevant at the start or stop of brodalumab.

What this interaction means in plain words

Brodalumab and Theophylline have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Brodalumab belongs to Miscellaneous; Theophylline belongs to Respiratory.

Evidence level: Low

Why it happens

Reduced cytokine-mediated suppression of CYP1A2 may increase theophylline metabolism.

What to do

Check serum theophylline concentrations and clinical response when brodalumab is started or stopped. Adjust the theophylline dose if levels change.
