Summary
Naltrexone blocks opioid receptors, so it antagonizes hydromorphone analgesia and can precipitate withdrawal in opioid-dependent patients. Hydromorphone is not primarily CYP2D6 dependent, so the pharmacokinetic effect of bupropion is minor by comparison.
What this interaction means in plain words
Bupropion-naltrexone and Hydromorphone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Bupropion-naltrexone belongs to Miscellaneous; Hydromorphone belongs to Musculoskeletal.
Why it happens
Naltrexone competitively antagonizes mu opioid receptors, opposing hydromorphone effects.
What to do
Avoid concurrent use; the product is contraindicated with chronic opioid therapy. Allow an adequate opioid-free interval before starting.
