Summary

Calcium carbonate can alkalinize urine, which increases renal elimination of salicylate. Salicylate levels may therefore fall, particularly at high anti-inflammatory doses, and efficacy may be reduced. The effect is generally modest with calcium carbonate compared with strong alkalinizing agents.

What this interaction means in plain words

Calcium carbonate and Salicylates have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Urinary alkalinization increases ionization of salicylic acid, reducing tubular reabsorption and enhancing renal excretion.

What to do

No action is usually needed with occasional antacid use; if salicylates are used at high doses with regular calcium carbonate, monitor therapeutic response or salicylate levels.

What to ask your pharmacist

  • Is there anything I should notice while taking both?
  • Do I need to tell my doctor at the next visit?
