Summary
Calcium salts can reduce the effect of calcium channel blockers in cases of high-dose or intravenous calcium, but oral calcium carbonate has little effect on amlodipine. Thiazides reduce urinary calcium excretion, so with large calcium carbonate doses there is a risk of hypercalcemia and milk-alkali syndrome.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Calcium carbonate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Calcium carbonate belongs to Gastrointestinal Tract.
Why it happens
Thiazide-reduced renal calcium excretion combined with calcium load, with minimal pharmacodynamic antagonism of amlodipine at oral doses.
What to do
Monitor serum calcium in patients taking regular or high-dose calcium supplements or antacids. Reduce calcium dose if hypercalcemia occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
