Summary
Oxaprozin can blunt the antihypertensive effect of candesartan and, together with an angiotensin receptor blocker, may impair renal function and raise potassium. The risk is greatest in elderly, volume-depleted or renally impaired patients.
What this interaction means in plain words
Candesartan and Oxaprozin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Candesartan belongs to Cardiovascular; Oxaprozin belongs to Musculoskeletal.
Why it happens
NSAIDs block vasodilatory prostaglandins that maintain glomerular perfusion while ARBs dilate the efferent arteriole, reducing glomerular filtration; both also promote hyperkalemia.
What to do
Ensure adequate hydration and monitor serum creatinine, potassium and blood pressure within the first weeks of combined use. Avoid or limit NSAID use in at-risk patients and consider alternative analgesics.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
