Summary
Salmeterol and hydrochlorothiazide can each lower serum potassium, and the effects may add together. Hypokalemia may provoke arrhythmias, particularly in patients with heart disease or those taking high doses of beta-agonist. The candesartan component partly offsets potassium loss.
What this interaction means in plain words
Candesartan-hydrochlorothiazide and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Candesartan-hydrochlorothiazide belongs to Cardiovascular; Salmeterol belongs to Respiratory.
Why it happens
Additive hypokalemia from beta-2 stimulated intracellular potassium shift and thiazide-induced renal potassium loss.
What to do
Monitor serum potassium, especially at initiation, with high doses or in patients with cardiac disease. Replace potassium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
