Summary
Cannabidiol inhibits CYP2C19, which helps convert clomipramine to its active metabolite, and also inhibits CYP2D6 to some extent. Clomipramine exposure may rise, increasing anticholinergic, sedative, seizure and cardiac adverse effects.
What this interaction means in plain words
Cannabidiol and Clomipramine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Cannabidiol belongs to Central Nervous System; Clomipramine belongs to Central Nervous System.
Why it happens
Inhibition of CYP2C19 (and possibly CYP2D6) by cannabidiol reduces clomipramine metabolism.
What to do
Monitor for tricyclic toxicity and consider lowering the clomipramine dose when cannabidiol is started. Check plasma levels or an ECG if symptoms or risk factors are present.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
