Summary
Arsenic trioxide causes marked QT prolongation and has been linked to torsades de pointes. Clomipramine also prolongs QT, so the combination adds serious arrhythmic risk.
What this interaction means in plain words
Arsenic trioxide and Clomipramine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Arsenic trioxide belongs to Oncology; Clomipramine belongs to Central Nervous System.
Why it happens
Additive QT prolongation by effects on cardiac ion channels.
What to do
Avoid the combination and use an antidepressant with minimal QT effect. If it cannot be avoided, use close ECG and electrolyte monitoring.
