Summary
Volatile anesthetics such as desflurane cause vasodilation and myocardial depression, and ACE inhibitors blunt compensatory angiotensin II vasoconstriction. Together they can produce pronounced hypotension during induction and maintenance of anesthesia.
What this interaction means in plain words
Captopril and Desflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril belongs to Cardiovascular; Desflurane belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic hypotensive effects, with ACE inhibition reducing the renin-angiotensin compensation for anesthetic-induced vasodilation.
What to do
Monitor blood pressure closely during anesthesia and treat hypotension with fluids or vasopressors. Consider whether to withhold the ACE inhibitor on the morning of surgery according to local perioperative protocol.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
