Summary
Starting an ACE inhibitor in a patient on a loop diuretic can cause marked first-dose hypotension and acute deterioration of renal function, especially with volume or sodium depletion. The combination is commonly and usefully prescribed, for example in heart failure, but requires care at initiation.
What this interaction means in plain words
Captopril and Furosemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril belongs to Cardiovascular; Furosemide belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic effect: diuretic-induced volume depletion activates the renin-angiotensin system, and ACE inhibition removes efferent arteriolar tone and vasoconstriction.
What to do
Consider temporarily lowering or holding the furosemide dose before the first captopril dose and start with a low dose. Monitor blood pressure, creatinine and potassium within the first one to two weeks.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
