Summary

ACE inhibitors can improve insulin sensitivity and may enhance the glucose-lowering effect of sulfonylureas, increasing the chance of hypoglycemia. Conversely, thiazides can raise blood glucose and blunt sulfonylurea effect, so glycemic control may become unpredictable. Chlorpropamide has a long half-life, so hypoglycemia may be prolonged.

What this interaction means in plain words

Captopril-hydrochlorothiazide and Chlorpropamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Captopril-hydrochlorothiazide belongs to Cardiovascular; Chlorpropamide belongs to Endocrine.

Evidence level: Moderate

Why it happens

ACE inhibitor enhancement of insulin sensitivity adds to sulfonylurea effect, while thiazide-induced hyperglycemia and potassium loss oppose it.

What to do

Monitor blood glucose closely when starting or stopping either agent and adjust the chlorpropamide dose as required. Counsel patients on hypoglycemia symptoms.

What to ask your pharmacist
