Summary
Fludrocortisone has strong mineralocorticoid activity that promotes potassium excretion, and hydrochlorothiazide also causes potassium loss. Together they can produce clinically relevant hypokalemia, and fludrocortisone also causes sodium retention that opposes the antihypertensive effect of the combination.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Fludrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Fludrocortisone belongs to Endocrine.
Why it happens
Additive renal potassium wasting, with mineralocorticoid-mediated sodium and fluid retention opposing antihypertensive effects.
What to do
Monitor serum potassium and blood pressure regularly, and supplement potassium or adjust doses as needed.
