Summary
Lofexidine is a central alpha-2 agonist that lowers heart rate and blood pressure. Combined with an ACE inhibitor and a thiazide diuretic, the blood pressure lowering effects add together and may cause symptomatic hypotension and bradycardia, including dizziness and syncope.
What this interaction means in plain words
Captopril-hydrochlorothiazide and Lofexidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Captopril-hydrochlorothiazide belongs to Cardiovascular; Lofexidine belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic hypotensive and bradycardic effects from central sympatholytic action plus RAAS inhibition and diuresis.
What to do
Monitor blood pressure and heart rate, especially during lofexidine initiation and dose changes, and reduce the antihypertensive doses if symptomatic hypotension develops. Counsel patients to rise slowly.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
