Summary
Carbamazepine is structurally related to tricyclic compounds, and combining it with an MAO inhibitor is labeled as contraindicated because of risk of hyperpyrexia, hypertensive crisis, and seizures. Transdermal selegiline at antidepressant doses carries this risk.
What this interaction means in plain words
Carbamazepine and Selegiline transdermal are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Additive pharmacodynamic effects of MAO inhibition with carbamazepine; the precise mechanism is not well established.
What to do
Avoid the combination; carbamazepine labeling requires a gap of at least 14 days after stopping an MAO inhibitor.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
