Summary

Carbidopa, usually given with levodopa, can add to the blood pressure lowering effect of antihypertensives such as nifedipine. Orthostatic hypotension and dizziness may appear when therapy is started or the dose is raised.

What this interaction means in plain words

Carbidopa and Nifedipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Carbidopa belongs to Central Nervous System; Nifedipine belongs to Cardiovascular.

Evidence level: Low

Why it happens

Additive hypotensive effect, mainly through levodopa-derived dopamine effects on vascular tone and autonomic regulation; carbidopa alone has little direct action.

What to do

Check supine and standing blood pressure when levodopa/carbidopa is started or titrated. Reduce the nifedipine dose if symptomatic hypotension develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
