Summary

Carbidopa is given with levodopa, and the dopaminergic therapy can add to the blood pressure lowering effect of ACE inhibitors. Symptomatic orthostatic hypotension may occur, especially when treatment is started or the dose is increased. Carbidopa alone has little effect.

What this interaction means in plain words

Carbidopa and Ramipril have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Carbidopa belongs to Central Nervous System; Ramipril belongs to Cardiovascular.

Evidence level: Low

Why it happens

Additive hypotensive effects, mainly from levodopa-derived dopamine, through pharmacodynamic synergism.

What to do

Monitor standing and sitting blood pressure when carbidopa/levodopa is started or adjusted, and reduce the ramipril dose if symptomatic hypotension develops.
