Summary
Droxidopa is converted to norepinephrine by dopa decarboxylase. Carbidopa inhibits this enzyme peripherally and can reduce droxidopa's pressor effect, while levodopa itself may also add to blood pressure variability.
What this interaction means in plain words
Carbidopa-levodopa and Droxidopa can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Carbidopa-levodopa belongs to Central Nervous System; Droxidopa belongs to Cardiovascular.
Why it happens
Carbidopa inhibits peripheral aromatic L-amino acid decarboxylase, reducing conversion of droxidopa to norepinephrine.
What to do
Monitor supine and standing blood pressure and symptoms of orthostatic hypotension. A higher droxidopa dose may be needed, within labeled limits, and watch for supine hypertension.
What to ask your pharmacist
