Summary
Nelfinavir inhibits CYP3A4 strongly enough to increase cariprazine exposure. Higher levels raise the likelihood of dose-related adverse effects such as akathisia, parkinsonism and sedation.
What this interaction means in plain words
Cariprazine and Nelfinavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Cariprazine belongs to Central Nervous System; Nelfinavir belongs to Anti Virals.
Why it happens
CYP3A4 inhibition decreases clearance of cariprazine and its active metabolites.
What to do
Reduce the cariprazine dose per labeling when combined with a strong CYP3A4 inhibitor, and monitor for movement disorders and sedation. Adjust again once nelfinavir is discontinued.
