Summary

Cefamandole can impair vitamin K-dependent clotting factor synthesis and platelet function, which may add to the anticoagulant effect of dalteparin. Bleeding risk may rise, particularly in malnourished, elderly or renally impaired patients.

What this interaction means in plain words

Cefamandole and Dalteparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive hemostatic effects: the N-methylthiotetrazole side chain of cefamandole causes hypoprothrombinemia and platelet dysfunction.

What to do

Monitor for bleeding and consider coagulation tests (PT/INR) and platelet count during co-therapy; give vitamin K if hypoprothrombinemia develops, or choose a cephalosporin without this side chain.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
