Summary

Defibrotide has profibrinolytic and antithrombotic activity, and combining it with a low molecular weight heparin adds to the risk of bleeding. Labeling for defibrotide contraindicates concurrent systemic antithrombotic or fibrinolytic therapy.

What this interaction means in plain words

Dalteparin and Defibrotide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dalteparin belongs to Cardiovascular; Defibrotide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic effects on hemostasis from anticoagulation plus enhanced fibrinolysis.

What to do

Do not give together; hold dalteparin around defibrotide therapy and use an alternative plan. Routine maintenance or reopening of central venous lines is the usual exception.
