Summary

Aminoglycosides are nephrotoxic and ototoxic, and combining them with a cephalosporin has been associated with additional kidney injury in some reports. Cefepime itself carries little direct nephrotoxicity, so the added risk is modest and mainly relevant in patients with existing renal impairment or prolonged therapy.

What this interaction means in plain words

Cefepime and Streptomycin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Cefepime belongs to Anti Bacterial; Streptomycin belongs to Anti Bacterial.

Evidence level: Low

Why it happens

Additive renal tubular stress; the aminoglycoside accumulates in proximal tubular and inner ear cells and any reduction in renal function raises its exposure.

What to do

Monitor serum creatinine and streptomycin concentrations, and adjust the dose to renal function. Watch for hearing or vestibular symptoms and limit the duration of combined therapy.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
