Summary

NSAIDs can blunt the antihypertensive effect of ARBs and diuretics and, with volume depletion, raise the risk of acute kidney injury (the so-called triple whammy). Hyperkalemia may also occur.

What this interaction means in plain words

Celecoxib and Hydrochlorothiazide-telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Celecoxib belongs to Musculoskeletal; Hydrochlorothiazide-telmisartan belongs to Cardiovascular.

Evidence level: High

Why it happens

Inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion while ARB and diuretic therapy impair glomerular autoregulation.

What to do

Use the lowest effective NSAID dose for the shortest time, ensure hydration, and monitor blood pressure, creatinine and potassium; avoid in elderly or volume-depleted patients when possible.

What to ask your pharmacist
