Summary
Combining an SSRI with an NSAID increases the risk of gastrointestinal and other bleeding compared with either drug alone. Paroxetine also inhibits CYP2D6, but celecoxib is mainly a CYP2C9 substrate so that is not the main concern.
What this interaction means in plain words
Celecoxib and Paroxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Celecoxib belongs to Musculoskeletal; Paroxetine belongs to Central Nervous System.
Why it happens
SSRIs deplete platelet serotonin and impair platelet aggregation, while NSAIDs add gastric mucosal injury and platelet effects.
What to do
Monitor for signs of gastrointestinal bleeding and consider gastroprotection with a proton pump inhibitor in older patients or those with prior ulcers. Use the lowest effective NSAID dose for the shortest duration.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
