Salicylates and some NSAIDs can enhance the glucose-lowering effect of sulfonylureas. Chlorpropamide has a long half-life, so hypoglycemia can be prolonged.
What this interaction means in plain words
Chlorpropamide and Diflunisal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Chlorpropamide belongs to Endocrine; Diflunisal belongs to Musculoskeletal.
Why it happens
Possible displacement of chlorpropamide from plasma proteins and reduced renal prostaglandin-mediated clearance, together with intrinsic salicylate glucose-lowering effects.
What to do
Monitor blood glucose more frequently when starting or stopping diflunisal, and reduce the chlorpropamide dose if hypoglycemia occurs. Counsel the patient on symptoms of hypoglycemia.
