Summary

ACE inhibitors can improve insulin sensitivity and enhance the glucose lowering effect of sulfonylureas, raising the risk of hypoglycemia. Chlorpropamide has a long half-life, so hypoglycemia may be prolonged. Hydrochlorothiazide can in contrast worsen glycemic control and also raises the risk of hyponatremia with chlorpropamide.

What this interaction means in plain words

Chlorpropamide and Enalapril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Chlorpropamide belongs to Endocrine; Enalapril-hydrochlorothiazide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

ACE inhibition enhances insulin sensitivity, adding to sulfonylurea hypoglycemic effect, while thiazides cause opposing hyperglycemia and additive hyponatremia.

What to do

Monitor blood glucose closely, especially after starting or changing the ACE inhibitor, and reduce the chlorpropamide dose if hypoglycemia occurs. Check serum sodium periodically.

What to ask your pharmacist
