Summary

Fluoxetine can enhance glucose-lowering and cause hypoglycemia in patients on antidiabetic drugs. Fluoxetine also inhibits CYP2C9, which metabolizes sulfonylureas, so chlorpropamide exposure may increase. Hypoglycemia can be prolonged because chlorpropamide has a long half-life.

What this interaction means in plain words

Chlorpropamide and Fluoxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Chlorpropamide belongs to Endocrine; Fluoxetine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Improved glycemic effects of SSRIs plus possible CYP2C9 inhibition reducing sulfonylurea clearance.

What to do

Monitor blood glucose closely when starting or stopping fluoxetine and lower the chlorpropamide dose if hypoglycemia occurs. Educate the patient on hypoglycemia symptoms.
