Summary

Guanfacine is a central alpha-2 agonist that reduces sympathetic outflow and may blunt the adrenergic warning symptoms of hypoglycemia such as tremor and palpitations. Patients on chlorpropamide, which causes prolonged hypoglycemia, may therefore fail to recognize low glucose.

What this interaction means in plain words

Chlorpropamide and Guanfacine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Chlorpropamide belongs to Endocrine; Guanfacine belongs to Cardiovascular.

Evidence level: Low

Why it happens

Reduced sympathetic and catecholamine-mediated hypoglycemia awareness through central alpha-2 agonism.

What to do

Counsel patients that sweating may remain but other warning signs can be reduced, and encourage regular glucose self-monitoring.
