Summary
NSAIDs can blunt the diuretic and antihypertensive effect of thiazide-type diuretics. Together they may also raise the risk of acute kidney injury, especially in volume-depleted or elderly patients.
What this interaction means in plain words
Chlorthalidone and Diflunisal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Chlorthalidone belongs to Cardiovascular; Diflunisal belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins causes sodium and water retention and reduced renal blood flow, opposing diuretic action.
What to do
Monitor blood pressure, fluid status, and renal function and electrolytes when the combination is used. Keep the patient well hydrated and adjust diuretic dose if control worsens.
What to ask your pharmacist
