Summary

Together these drugs give additive blood pressure lowering and are used intentionally. Chlorthalidone-induced hypokalemia is not reliably offset by nadolol. Nadolol is renally cleared, so diuretic-induced volume depletion can raise its levels and increase bradycardia or hypotension.

What this interaction means in plain words

Chlorthalidone and Nadolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Chlorthalidone belongs to Cardiovascular; Nadolol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive antihypertensive effects; reduced renal perfusion may decrease nadolol clearance.

What to do

Monitor heart rate, blood pressure and electrolytes, and reduce the nadolol dose if bradycardia or hypotension develops.
