clopidogrel + pantoprazole

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Pantoprazole may modestly reduce clopidogrel’s antiplatelet effect; effect is smaller than with omeprazole/esomeprazole but potential reduction in active metabolite and platelet inhibition can occur.

Mechanism of action

Weak inhibition of CYP2C19 by pantoprazole may decrease bioactivation of clopidogrel to its active thiol metabolite, reducing platelet inhibition.

Clinical management

Use only if gastroprotection is clearly indicated; prefer pantoprazole over omeprazole/esomeprazole. Use lowest effective PPI dose and monitor for recurrent ischemic events or stent thrombosis. Consider H2RA (not cimetidine) if appropriate.
