Summary

Conivaptan is a potent CYP3A4 inhibitor and can markedly raise lurasidone exposure. Higher levels increase the risk of sedation, extrapyramidal symptoms, akathisia and other dose-related adverse effects. Lurasidone labeling contraindicates strong CYP3A4 inhibitors.

What this interaction means in plain words

Conivaptan and Lurasidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated metabolism of lurasidone in the liver and gut wall.

What to do

Avoid the combination; if conivaptan is needed, consider an alternative antipsychotic or an alternative treatment for hyponatremia.
