Summary
Darunavir, usually given with ritonavir or cobicistat, strongly inhibits CYP3A4 and can substantially increase lurasidone concentrations. This raises the risk of dose-related adverse effects such as extrapyramidal symptoms, sedation and akathisia. Lurasidone labeling contraindicates strong CYP3A4 inhibitors.
What this interaction means in plain words
Darunavir and Lurasidone are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Darunavir belongs to Anti Virals; Lurasidone belongs to Central Nervous System.
Why it happens
Inhibition of CYP3A4-mediated first-pass and systemic metabolism of lurasidone.
What to do
Do not coadminister; choose an antipsychotic less dependent on CYP3A4 metabolism, or use an alternative antiretroviral.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
