Summary
Cyclosporine inhibits hepatic uptake transporters OATP1B1 and OATP1B3, which can raise systemic exposure to the active metabolite of inhaled revefenacin. Higher exposure could increase anticholinergic adverse effects such as dry mouth, urinary retention and tachycardia.
What this interaction means in plain words
Cyclosporin and Revefenacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Cyclosporin belongs to Miscellaneous; Revefenacin belongs to Respiratory.
Why it happens
Cyclosporine inhibits OATP1B1/OATP1B3-mediated uptake, reducing hepatic clearance of revefenacin's active metabolite.
What to do
Coadministration is not recommended per labeling; choose an alternative long-acting bronchodilator if cyclosporine is needed. If unavoidable, monitor for anticholinergic effects.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
