Summary

Heparins, including low molecular weight heparins, can suppress aldosterone secretion and raise serum potassium. Together with enalapril, which also reduces aldosterone, hyperkalemia is more likely, especially with renal impairment or diabetes.

What this interaction means in plain words

Dalteparin and Enalapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dalteparin belongs to Cardiovascular; Enalapril belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive suppression of aldosterone by heparin and ACE inhibition, leading to potassium retention.

What to do

Monitor serum potassium and renal function, particularly if treatment lasts more than a few days. Stop or reduce potassium-raising drugs if hyperkalemia develops.

What to ask your pharmacist

  • Does the dose of either medicine need to change?
