Summary

Heparins can suppress aldosterone production by the adrenal gland, which may reduce potassium excretion. Combined with an angiotensin receptor blocker such as olmesartan, this can raise serum potassium, particularly in patients with kidney disease or diabetes. The thiazide component partly offsets this effect.

What this interaction means in plain words

Dalteparin and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dalteparin belongs to Cardiovascular; Hydrochlorothiazide-olmesartan belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced suppression of aldosterone synthesis adds to angiotensin receptor blockade, reducing renal potassium excretion.

What to do

Check serum potassium and renal function at baseline and periodically if dalteparin is given for more than a few days. Pay particular attention to patients with renal impairment, diabetes or other hyperkalemia risk factors.

What to ask your pharmacist
