Summary

Parecoxib, a prodrug of the COX-2 inhibitor valdecoxib, has less effect on platelets than non-selective NSAIDs but can still cause gastrointestinal and renal problems. Combined with dalteparin the bleeding risk may be modestly increased.

What this interaction means in plain words

Dalteparin and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

COX-2 inhibition spares platelet thromboxane but may impair mucosal protection and renal function, adding to bleeding risk with dalteparin.

What to do

Use the lowest effective parecoxib dose for the shortest time and monitor for bleeding. Check renal function, since impairment increases dalteparin accumulation.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
