Summary

Heparins, including low molecular weight heparins, can suppress aldosterone secretion and cause hyperkalemia. ACE inhibitors also reduce aldosterone, so the effects add up, especially with renal impairment or diabetes.

What this interaction means in plain words

Dalteparin and Perindopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dalteparin belongs to Cardiovascular; Perindopril belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin-induced inhibition of adrenal aldosterone synthesis adds to ACE inhibitor-mediated reduction of aldosterone, impairing potassium excretion.

What to do

Monitor serum potassium and renal function, particularly with prolonged dalteparin use or in high-risk patients. Treat hyperkalemia or review therapy if potassium rises.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
