Summary

Heparins can suppress aldosterone secretion, and ACE inhibitors also lower aldosterone, so combined use can raise serum potassium. Risk is higher in diabetes, renal impairment and prolonged heparin use.

What this interaction means in plain words

Dalteparin and Quinapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive reduction in aldosterone-mediated potassium excretion, with heparin inhibiting adrenal aldosterone synthesis and quinapril reducing angiotensin II.

What to do

Check serum potassium and renal function at baseline and periodically during extended co-therapy, particularly in high-risk patients; manage hyperkalemia if it appears.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
