Summary
Dronedarone is both a CYP3A4 substrate and a moderate CYP3A4 and a CYP2D6 inhibitor, and darifenacin is mainly a substrate of these enzymes. Darifenacin exposure may rise, and dronedarone exposure may be affected only minimally. The clinical significance is chiefly increased anticholinergic effects.
What this interaction means in plain words
Darifenacin and Dronedarone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Darifenacin belongs to Genitourinary; Dronedarone belongs to Cardiovascular.
Why it happens
Dronedarone inhibits CYP2D6 and CYP3A4 and may raise darifenacin levels; darifenacin is a weak CYP2D6 inhibitor with little effect on dronedarone.
What to do
Monitor for anticholinergic effects and limit darifenacin to 7.5 mg daily if they occur. Check the ECG for QT changes.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
