Summary
Erythromycin is a moderate CYP3A4 inhibitor and can raise dasatinib exposure, increasing the risk of myelosuppression, fluid retention and pleural effusion. Both drugs also prolong the QT interval, so the combination adds arrhythmia risk. Dasatinib has weak CYP3A4 inhibitory effects, so any rise in erythromycin levels is of lesser importance.
What this interaction means in plain words
Dasatinib and Erythromycin base can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Erythromycin inhibits CYP3A4-mediated metabolism of dasatinib, and both agents have additive QT-prolonging effects.
What to do
Prefer a non-interacting antibiotic such as azithromycin with caution or a non-macrolide alternative. If unavoidable, monitor ECG, electrolytes and dasatinib toxicity, and consider reducing the dasatinib dose.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
