Summary
Mycophenolate suppresses T- and B-lymphocyte proliferation and may blunt the antibody response to dengue vaccination. The dengue vaccine is a live attenuated product, so immunosuppressed patients also face a theoretical risk of vaccine-related infection. Protection may therefore be incomplete and the vaccine may be unsafe.
What this interaction means in plain words
Dengue vaccine and Mycophenolate mofetil can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Pharmacodynamic antagonism: inhibition of lymphocyte proliferation reduces vaccine immunogenicity, and live vaccine replication is less well controlled.
What to do
Avoid giving the live dengue vaccine to patients on mycophenolate at immunosuppressive doses; consult the product label and the transplant or specialist team. If vaccination is essential, consider timing it before immunosuppression begins.
