dexamethasone + diclofenac

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: High

Systemic corticosteroids with NSAIDs increase risk of GI ulceration/bleeding/perforation; may also raise BP/edema and renal risk, especially with higher doses or prolonged use.

Mechanism of action

Additive GI mucosal injury: NSAID COX-1 inhibition reduces protective prostaglandins; glucocorticoids impair mucosal defense and healing; both may promote fluid retention and reduce renal perfusion.

Clinical management

Avoid if possible. If necessary, use lowest effective doses/shortest duration; add PPI gastroprotection in at-risk patients; monitor for GI bleeding, BP/edema, and renal function (SCr) during ongoing therapy.
