Summary
Fluoroquinolones and systemic corticosteroids together increase the likelihood of tendinitis and tendon rupture, most often the Achilles tendon. The risk is greater in older adults, transplant recipients and those on prolonged steroid courses.
What this interaction means in plain words
Dexamethasone and Levofloxacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dexamethasone belongs to Endocrine; Levofloxacin belongs to Anti Bacterial.
Why it happens
Both drugs impair tendon collagen integrity and repair, producing additive tendon toxicity.
What to do
Avoid the combination in older patients or those on long-term steroids when other antibiotics are suitable. Counsel patients to stop levofloxacin and rest at the first sign of tendon pain or swelling.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
