Summary
Ritonavir strongly inhibits CYP3A4 and can markedly raise dexamethasone exposure, causing systemic corticosteroid effects including Cushing syndrome and adrenal suppression with prolonged use. Dexamethasone, in turn, may induce CYP3A4 and lower ritonavir concentrations, potentially reducing antiviral efficacy.
What this interaction means in plain words
Dexamethasone and Ritonavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dexamethasone belongs to Endocrine; Ritonavir belongs to Anti Virals.
Why it happens
Ritonavir inhibits CYP3A4 and P-glycoprotein, increasing dexamethasone exposure, while dexamethasone induces CYP3A4 and may reduce ritonavir levels.
What to do
Avoid prolonged combined use where possible, or reduce the dexamethasone dose and monitor for corticosteroid toxicity. Monitor HIV viral load and consider alternative corticosteroids such as beclomethasone or prednisolone for long-term therapy.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?

Frequently asked questions
Can I take Dexamethasone with Ritonavir?
MediHelp rates the interaction as major. Dexamethasone and Ritonavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
How serious is the interaction between Dexamethasone and Ritonavir?
MediHelp rates it major, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Avoid prolonged combined use where possible, or reduce the dexamethasone dose and monitor for corticosteroid toxicity. Monitor HIV viral load and consider alternative corticosteroids such as beclomethasone or prednisolone for long-term therapy.
