Summary

Systemic corticosteroids combined with a non-depolarizing neuromuscular blocker, particularly in prolonged ICU use, have been linked to acute quadriplegic myopathy and delayed recovery of strength. This may prolong mechanical ventilation.

What this interaction means in plain words

Dexamethasone and Vecuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dexamethasone belongs to Endocrine; Vecuronium belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive myotoxic effects of corticosteroids and prolonged neuromuscular blockade; the exact mechanism is not fully established.

What to do

Limit vecuronium dose and duration and guide dosing with train-of-four monitoring. Evaluate for prolonged weakness and check creatine kinase if myopathy is suspected.

What to ask your pharmacist
