Fluoroquinolones combined with systemic corticosteroids increase the risk of tendinitis and tendon rupture, especially in older patients. The risk is much lower with topical dexamethasone, but the combination is relevant when systemic steroids are used.

What this interaction means in plain words

Dexamethasone-tobramycin and Gemifloxacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Dexamethasone-tobramycin belongs to Anti Bacterial; Gemifloxacin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Additive tendon toxicity from fluoroquinolone effects on tenocytes and corticosteroid-related collagen impairment.

What to do

Avoid combining where alternatives exist, especially in patients over 60 or with transplants; stop the fluoroquinolone and rest the limb at the first sign of tendon pain.
