Summary

Ibutilide is a potent QT-prolonging class III antiarrhythmic with a high torsades risk. Gemifloxacin adds to the effect and may increase the chance of life-threatening arrhythmia.

What this interaction means in plain words

Gemifloxacin and Ibutilide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Gemifloxacin belongs to Anti Bacterial; Ibutilide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive potassium channel blockade prolonging ventricular repolarization.

What to do

Avoid giving gemifloxacin to patients receiving ibutilide, and use a non-QT-prolonging antibiotic. If an antibiotic is essential, ensure continuous ECG monitoring with cardiology input.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
